      • A/T Details

      • DD slash MM slash YYYY
        The date of which you conducted the exit interview.
      • DD slash MM slash YYYY
        The date in which the A/T cancelled or resigned.
      • What qualification were they enrolled into
      • For example: carpentry apprentice, warehousing trainee, etc.
      • Which host employer were they working for
      • Exit Interview Questions

        To be asked in order, along with probing questions to get more details where needed. Before asking questions, introduce yourself and what your role is, what is the purpose of the call (exit interview), ask if they have time for interview and emphasis that this information is being collected as feedback as a way to improve our services.
      • Ask them if it was one big instance or a combination of smaller things over a period of time?
      • If yes, was there any alternative resolution? If no, was there any reason why you didn't bring it up?
      • Full-time work, have a break, study?
      • If yes, how so?
      • Was it adequate? Did you feel as if you were properly trained? How could it be improved?
      • Only ask if they are obtainable changes. For example, if they're a 1st year apprentice and ask for $25 an hour, that probably isn't very obtainable.
      • Emphasis to them that all feedback is valuable and they should not be afraid to be honest as we are trying to improve our services for the future.
